A couple years ago I had a tutorial on how to create a customer chore chart using my Chore Chart kids. Some how it got deleted so I am writing up the tutorial again for those interested. My son has outgrown his chart but it sure did come in handy when he was in elementary school.
Items Needed:
This chart is meant to fit into a baseball or hockey card protector. You know the kind that have 9 slots for cards and you keep in a binder. I picked mine up at the dollar store. For this project I used two protectors. 
To start cut triangles out of the side of the sleeve that opens. This will allow your child to add/remove the finished tags more easily. It also makes changing out the cards easier. You don't need to do this for the header slots but you can. I created a template to cut along so they all looked the same. Each pocket is approximately 2.5 x 3.5 inches. It looks like this:
